Terms & Conditions

I hereby authorize AGENT_NAME to initiate a direct payment from my credit card or designated bank account for the total amount specified on this invoice. I understand that this payment is for travel services provided by AGENT_NAME as outlined in the corresponding booking confirmation.

I understand that this authorization will remain in effect until I provide written notification of cancellation or modification to AGENT_NAME.

I understand that returns, refunds, and cancellations are at the discretion of Addy and Terry Travel. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company, so long as the transaction corresponds to the terms indicated on this invoice.

By making this payment, I acknowledge and agree to the following terms and conditions as previously agreed to in the Travel Services Agreement.

I certify that I am the authorized cardholder and that I have read and understand the terms and conditions outlined above.
