- A Passport is required to enter Punta Cana, Dominican Republic and cannot expire on or before 01/06/2027. If your passport expires before this date you must get is renewed.

🚫 -Flights are NOT included but can be added. Deposit must be PAID to secure your reservation, and prices are subject to change until deposit or full payment applied is applied. This will change the amount of your deposit as well.

- Airfare- Each guest is responsible for booking their own flights and submitting a transportation form to me which will have all flight details. Flights typically become avail 331 days before the trip.

- Deposits are $100.00 per person and are non-refundable whether travel insurance is purchased or not.

-Agent does charge a $50.00 per person planning service fee that is added to package.

- A monthly payment schedule will be based upon the amount of the room & the final payment due date.

 -Vacation packages NON-TRANSFERRABLE and ONLY REFUNDED through your travel insurance provider and NOT Melanin Ultimate Travels. If you purchase travel insurance, your trip may be refundable through that avenue only. (see terms and conditions of your insurance provider).

 -Travel Insurance is not  included in this package, but can be added and  purchasing is highly recommended. You may purchase your own but must be purchased with 21 days of booking. If purchasing your own travel insurance client must submit proof of travel insurance to agent via email:Teresa@melaninultimatetravels.com.

-You may choose your own travel insurance carrier, or you may visit my website www.melaninultimatetravels.comand click travel insurance to purchase.

-All final payments are due no later than 05/15/2026. No Exceptions. 

- Name changes are allowed with a $50 name change fee up until the final payment date.

- All travel documents will be sent 14-21 days prior to departure.

 -All cancellations and/or changes must be received in writing via email only to:Teresa@melaninultimatetravels.com.

 -All cancellations will incur a $50 cancellation fee.

- This Credit Card Authorization will be placed on file. This form is to book only your hotel, and flight, travel protection, transportation and any group excursions that maybe included in accommodations.

*I agree to submit a new credit card authorization form if I must change my credit card information at anytime before  the final payment is completed.

 * I agree that if the payment cannot be drafted, I will contact Melanin Ultimate Travels(formally Melanin Ultimate Travel) immediately to provide another payment method. 

I understand that travel insurance is NOT included in  my package, but can be added, and if I choose to get my own travel insurance that I have 21 days from the date the trip was secured to purchase travel insurance. MELANIN  ULTIMATE TRAVELS  DOES NOT issue refunds unless provided within the package from the travel partner.  In the event of a cancellation, I will use the third party travel insurance provider as a means to receive any refunds(see terms and conditions of the travel insurance provider.)

I understand that I may see charges from Melanin Ultimate Travels and their Travel Partners and that records will be provided to me to confirm that I submitted or authorized payment(s).

NOTE: Your card may not be automatically  charged by completing and submitting this form; however, completing and submitting this form does give the agent the right to charge the card for the amount and purpose authorized above.

I, the above-named authorized credit card user, give Melanin  Ultimate Travels  and/or it's Travel Partners(vendors) authorization to charge my credit card for the purposes detailed above. I understand that this form is a legally binding contract and that by affixing my signature to this form, I will be held responsible for all agreed upon charges. This credit card is authorized for only the charges noted above. 

 This payment is for the services described on the invoice. I understand that all payments made to Melanin Ultimate Travels  are non-refundable. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company, so long as the transaction corresponds to the terms indicated on this invoice.
