I authorize MAC Group Travel, LLC to charge the credit card indicated on this invoice for the noted amount on today's date. This payment is for the services described on the invoice. I understand that all payments made to MAC Group Travel, LLC are non-refundable, unless otherwise stated in the Cancellation Policy above. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company, so long as the transaction corresponds to the terms indicated on this invoice.

I understand that travel insurance is NOT being offered by MAC Group Travel, LLC and could result in the loss of my travel cost and/or require more money to correct the situation. I acknowledge I am able to purchase travel insurance through another independent agency at my own cost.  I also acknowledge that without insurance, there may be no way to recoup any losses, costs or expenses incurred.
