In order to create your reservation for The Alexander-Hurts Wedding in September 2022, we will need to capture a few personal details. The information requested in the required fields is the minimum details needed to create your reservation to lock in pricing. If the second traveler on the reservation is paying separately, the second traveler will need to submit their own individual registration form. If children will be added to the booking, please provide their full names and date of birth in the space provided. The invoice will be updated to reflect additional child pricing with the applicable deposit due.

 
 

IMPORTANT NOTES ABOUT COMPLETING YOUR RESERVATION:

  1. Please enter your legal name(s) EXACTLY as appears on your valid passport. 

    • Do not use nickname and include middles names, if listed on your passport.

  2. Your Passport Number is NOT required at time of booking but will be needed prior to final payment

    • Passports are REQUIRED for all international travel via air or cruise.

    • Your passport must not expire within 6 months of travel.  You may leave the passport number and expiration date blank below, but please do not forget to make sure it’s not expired.

  3. Every field marked with an asterisk (*) is Required and you will not be able to submit the form without ALL mandatory (*) fields properly completed.

  4. Additional fees may apply if a reservation has to be changed due to incorrect information. Felecia Storey / Fab World Travel Designs is not liable if the information below is not entered correctly!

  5. Submitting information via this reservation form does not automatically confirm your reservation or charge your debit/credit card.

  6. Your submitted payment will be processed & email confirmation with the invoice will be sent out within 48hrs. Please note an automatic email is not generated by the system. Once submitted you will see a small pop-up box confirming your submission.

  7. It is your responsibility to review the invoice upon receipt to ensure 100% accuracy. Please check thoroughly for any discrepancies or errors. Fab World Travel Designs is NOT responsible for errors, or any fees associated with necessary changes after 24hrs of receipt.
