I hereby verify that all names are spelled correctly and are legal as printed on a government-issued identification card. I hereby affirm that I have reviewed my travel itinerary for accuracy of dates, hotels, locations, etc. I have been advised of all applicable fees imposed by Treat Yourself, the suppliers, and any scheduled fees that may be required if this booking is canceled or changed.

I certify that I am an authorized user of this credit card, and I will not dispute the charge with my credit card company so long as the transaction corresponds to the terms indicated on this form. This payment is for the services described on the invoice. I understand that all payments made to Treat Yourself Travel are non-refundable.

By purchasing a trip from Treat Yourself Travel, I acknowledge, and I am aware that the trip details are subject to change. The itinerary was curated in good faith. However, any mention of specific destinations does not guarantee they will be visited, nor does it forfeit the contract between the purchaser and the seller. Furthermore, I agree and understand that Treat Yourself Travel is not responsible for any operating failures, loss, damage, delay, or injury to me or members of my traveling party, nor for consequential damages that may result from failure, delays, or injuries, which arise out of or during the scheduled travel. I also acknowledge and agree that I inherent all risks that are associated with any activities I may partake in. Therefore, I understand, agree, and assume full responsibility for the risk I alone am responsible for based upon my decision to participate in such activities. Additionally, I agree and understand that Treat Yourself Travel is not liable for any damages, direct or consequential, which may arise as a result of acts of God, social or labor unrest, war, political strike, terrorist activities, mechanical failures, climatic conditions, or any other actions or omissions beyond the control of Treat Yourself Travel. All payments to Treat Yourself Travel are non-refundable and non-transferable. No refunds or transfers will be provided after reservation confirmation, regardless of cancellation or changes.

In addition, I have been informed auto-pay is required as well as of the cancellation policies for my travel services and have been made aware of the benefits of travel protection.
