GENERAL CONDITIONS 
1. ORGANIZATION. The travel is organized and provided by International Consultants/Fashion For Futures with an on-site partnership and provides a tutoring service on site as well. 

2. COST AND PAYMENT. The price includes everything as described in the travel sample. Since Fashion For Futures is committed to researching the best offers for travel, accommodation, study programs, activities and food, the final price will be communicated only when registrations are concluded and before hotel bookings. Any discounts in case of early bookings and/or promotions will be applied to the final price. Any additional costs in case of changes by the customer will be communicated as soon as possible. Fashion For Futures reserves the right to cancel the trip if the minimum number of participants is not reached (a minimum of 8 persons per trip).. However, cancellations made by the student/s are not refundable. Cancellations made by International Consultants/Fashion For Futures will be reimbursed for the confirmation of booking fee and the deposit payments. DEPOSIT PAYMENTS ARE NON REFUNDABLE. 

3. INSURANCE. Medical insurance is covered solely by the undersigned participant. Any damage caused by students on site is not covered by insurance: by signing this form the student expressly declares to renounce any civil or criminal action or recourse against International Consultants, operators and collaborators for direct and indirect damages that might occur during the trip. 

DECLARATION OF LIABILITY 

1. By signing this PARTICIPATION FORM, the undersigned declares under his own responsibility to be in good physical condition and to exonerate and release International Consultants, the teacher, the staff and the organizers, the lawyer, the collaborators, from any civil and criminal liability, occurring during the stay, as well as, by way of example but not limited, the owners and/or managers of the facilities, the managerial and employee staff assigned to the planned activities, for any accidents suffered, illnesses etc. 

2. The student hereby undertakes to behave in compliance with the principles of loyalty and correctness, decency and common sense during the trip in question and to respect the Italian and the American law. and in any case not to endanger its own or others' safety. 

3. The student also declares to waive any request for compensation and reimbursement that could be claimed against International Consultants, its legal representative as well as the operators and collaborators of the company and therefore exempts International Consultants from any liability and any type of judicial and/or arbitration proceedings relating to the risk of accidents, compensation for damage to persons and/or property of third parties, damage. 

4. The student declares to exonerate International Consultants and its legal representative from any responsibility for any loss/theft, damage and expenses, that the student or his family members suffer from a non-compliant behavior. 

5. The student declares to be aware that the rules related to the containment of COVID19 are mandatory in the countries where the activities foreseen by the present trip take place and therefore undertakes, under penalty of exclusion from the activities of the offer, to learn and respect them in every situation/event during the trip (as an example but not limited: use of masks, swabs, temperature detection, social distancing etc..)

6.  Please be aware that participating in the two-day event, specifically working backstage at Fashion Week, will involve intense physical activity. By completing this form, you are confirming that you are physically capable of undertaking these demanding tasks. Please confirm. 

Permission and Release form for images, videos and sound recordings.

 I declare that I am aware that International Consultants may use the data contained in this self certification exclusively within its own institutional purposes (Law 196/2003 "Protection of privacy"). I am also aware that the activities of the International Consultants could be the subject of photo and press services, radio-television tapes, video-recordings and therefore I authorize the aforementioned services, which could also concern my person and image without reservation to such use, for the promotion of International Consultants, either by its partners or by third parties authorized by the Association as part of its promotional activities. This authorization is granted in full freedom and autonomy, without conditions or reservations and completely for free. 

FINAL STATEMENT 
The undersigned, aware that the submission of untruthful declarations involves criminal sanctions provided for by art. 76 of Presidential Decree 28 December 2000, n. 445 declares that the data reported on this form are complete and truthful. 
Pursuant to art. 1341 and 1342 of the Civil Code, the undersigned declares to have carefully read and approved this document. 
I, the undersigned, authorize the processing of my personal data, pursuant to Legislative Decree 196/2003, for the purposes of managing the entire trip.
