You certify that you have verified that all of the information contained in the confirmation that you received is accurate. You also certify that you have read the Terms & Conditions and protection plans and adhere to the cancellation penalties as outlined.
The charge as authorized will be applied manually by the agency to your said reservation. If there are any issues with payment, an agent will get back to you.
I authorize Purposely Made Travel, acting as an independent agent under LH Fun Travel, LLC to use the above information to charge my credit card the amount stated. Completion of this form along with the signature signifies acceptance of this reservation.
