To complete your transaction and confirm your arrangement, your signature on this authorization form is required. I authorize Opulence Travel Services and or Tamarian Maddox and or the travel supplier to charge the credit card indicated on this invoice for the noted amount.
I certify that I am aware that ALL payments to Opulence Travel Services and or Tamarian Maddox and or the travel supplier are NON-REFUNDABLE and NON-TRANSFERABLE.
Cancellation Fee.
In the event of any trip cancellation, there will be a cancellation fee up to $100 per person required for Tamarian Maddox subtracted from the refund.
I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company or bank, so as long as the transaction corresponds to the terms indicated on this invoice.
TRAVEL INSURANCE WAIVER
For your protection, Travel Insurance is strongly recommended and available upon request. You can enroll online for travel protection for Medical Expenses, Baggage Delays/Loss, Trip Delay or Cancellation, and other coverage, or your Independent Travel Agent can arrange coverage for you, or you may use a third party travel insurance.
I have read and understand all cancellation charges and change fees related to the travel arrangements, and that I may not be entitled to a refund should my travel plans change if Travel Insurance is not purchased. In case of cancellation of nonrefundable airline tickets or other arrangements, I agree to pay all applicable penalties according to the travel supplier's rules.
