We are 5 Star Travel and we are excited to help you plan your trip. Please review the following terms & conditions (the "Agreement") that will apply. 
By providing your name below and selecting the "Submit" button, you are signing this agreement electronically. You agree that your electronic signature is the legal equivalent of your manual signature on this agreement. By selecting the "submit" button, you consent to be legally bound by this agreement's terms and conditions.
1. I/We acknowledge that all the information above is correct. By signing this form, I am giving authorization to Lashawne Kent-Matthews of 5-Star Travel, LLC, Oasis Travel Network, and/or our travel suppliers to charge my card for all charges listed above
2. I/We understand and accept the Booking Terms, Cancellation Penalties and Conditions detailed in your proposal
3. I/We are aware valid passports, visas and health documents may be required. Obtaining these documents is my/our responsibility (Note: some countries require that you have a valid passport for at least 6 months after your date of return)
4. I/We agree and understand that 5-Star Travel, and its agents can accept no responsibility for losses or accidental expenses due to delays or changes in airline/rail or other means of transport schedule, hotel over-booking or default, sickness, weather, strikes, war, quarantine, or other causes.  Therefore, Travel Protection Insurance is recommended.
