By signing below, I authorize LH Fun Travel, LLC to charge the credit card provided herein to its chosen Tour Operator/Supplier/Cruise Line with the listed amount. You certify that you have verified that all of the information contained in the confirmation that you received is accurate. You also certify that you have read the Terms & Conditions and protection plans and adhere to the cancellation penalties as outlined. 

The full deposit as required by our third party vendor is NON REFUNDABLE for cancellations. Insurance/waiver costs are also non-refundable. Modifications will incur a $100 per person fee. Only when applicable, a flight name change will receive a $200 per person name change fee. A hotel only name change fee will be $100 per person. 

The charge as authorized will be applied manually by the agency to your said reservation. If there are any issues with payment, an agent will get back to you. Please note that the charge will directly reflect the supplier's name in which your reservation was created.

I authorize LH Fun Travel, LLC to use the above information to charge my credit card the amount stated. Completion of this form along with the signature signifies acceptance of this reservation.
