By signing below, I authorize LH Fun Travel, LLC to charge the credit card provided herein for the initial non-refundable deposit in the amount listed. I further authorize subsequent scheduled payments to be charged directly by the applicable Tour Operator, Supplier, Cruise Line, Hotel, or Travel Vendor associated with my reservation, in the amounts and on the dates agreed upon. I certify that I have verified that all of the information contained in the confirmation that I received is accurate. I also certify that I have read the Terms & Conditions and protection plans and adhere to the cancellation penalties as outlined.

Deposit and Payments: The deposit is non-refundable. If cancellation occurs prior to final payment, a future travel credit will be issued for all payments made excluding the deposit and must be used within one year of the cancellation date. All payments become non-refundable and non-transferable after final payment. Travel insurance is HIGHLY RECOMMENDED!! Protect your vacation investment TODAY by purchasing travel insurance! *The total cost is dependent on a group travel rate and are subject to change if the participation falls below a number consistent on a group. I further acknowledge that I have been advised to purchase travel insurance to protect my investment. 
Tours: Please note that some tours are included and are subject to change due to weather and or unforeseen circumstances. We will do our best to supply another tour to take the place of the tour that was replaced. Tours do not have a monetary value for a cash or credit refund. 

The charge as authorized will be applied manually by the agency to your said reservation. If there are any issues with payment, an agent will get back to you. Please note that the charge will directly reflect the supplier's name in which your reservation was created.

I authorize LH Fun Travel, LLC to use the above information to charge my credit card the amount stated. Completion of this form along with the signature signifies acceptance of this reservation.
