This form is NOT required for any purchases you complete yourself on our websites, or its affiliates. Charges are payable ONLY to the hotel or resort, tour operator, cruise line, or other travel suppliers. I do not accept or process charges through any other account, nor do I accept cash, check, or any other forms of payment. 

By signing below, I have ACCEPTED and REVIEWED all travel arrangements, and therefore authorize the travel purchases as stated above, and I am aware the insurance premium is non-refundable if I choose to cancel.

Terms & Conditions

I authorize KatsKreations Travel & Tours to charge the credit card or bank account indicated on this invoice for the noted amount on today's date. This payment is for the services described on the invoice. I understand that returns, refunds, and cancellations are at the discretion of the travel supplier or tour operator used. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company, so long as the transaction corresponds to the terms indicated on this invoice.  You (the client, and your traveling party) are responsible for all of the information contained in these terms, even if you do not review it.
