By submitting this form and/or making payment toward your reservation, you acknowledge and agree to the following terms and conditions:

1. Identification Requirements
   All travelers are required to provide a valid government issued photo ID or passport that matches the name on the reservation exactly. Acceptable forms of identification include a driver’s license, state ID, military ID, or passport. Travelers are responsible for ensuring all travel documents are current and valid for travel.
2. Payment Authorization
   By submitting payment information, you authorize E & J Travel and/or the applicable travel supplier to charge the payment method provided for authorized travel purchases and related services associated with your reservation.
3. Travel Insurance Recommendation
   Travel insurance is strongly recommended for all travelers and is available upon request. Travel insurance may provide coverage for trip cancellation, trip interruption, medical expenses, baggage delays or loss, and other unforeseen circumstances.

• • If you choose to decline travel insurance, you acknowledge that you understand the risks associated with traveling without coverage and accept responsibility for any cancellation penalties, change fees, supplier penalties, medical expenses, travel interruptions, or other financial losses that may occur.

1. Cancellation and Change Fees
   Cancellation policies and change fees vary by supplier, cruise line, hotel, airline, and tour operator. Some deposits and payments may be nonrefundable. By submitting this form, you acknowledge that you have read and understand that penalties may apply should your travel plans change. In the case of cancellation of nonrefundable airline tickets or other travel arrangements, all applicable supplier penalties and fees will apply.
2. Pricing and Availability
   All rates, promotions, and travel arrangements are subject to availability, supplier restrictions, and change until deposited and confirmed. Pricing is not guaranteed until payment has been processed and confirmation has been issued.
3. Responsibility for Travel Documents
   It is the responsibility of each traveler to verify passport requirements, visa requirements, vaccination requirements, REAL ID compliance, and any travel restrictions prior to departure. E & J Travel is not responsible for denied boarding or travel interruptions due to invalid or missing documentation.
4. Agency Responsibility
   E & J Travel acts only as an independent travel advisor and agent for travel suppliers including airlines, cruise lines, hotels, resorts, and tour operators. E & J Travel is not responsible for acts, omissions, injuries, delays, cancellations, supplier defaults, weather events, mechanical failures, government actions, or any other events beyond its control.
5. Credit Card Security and Verification
   For your protection, E & J Travel may require a signed authorization form and a legible copy of the front and back of the credit card used for payment prior to processing travel arrangements.
6. Privacy Policy
   Personal information collected through this form is used solely for the purpose of processing travel arrangements and communicating regarding your reservation. Information will only be shared with travel suppliers and payment processors as necessary to complete your booking.
7. Consent to Communication
   By submitting this form, you authorize E & J Travel to communicate with you via phone call, text message, and email regarding your reservation, payment reminders, travel updates, and important travel notifications.
