I authorize Lez Go Travel to charge the credit card indicated on this invoice for the amount listed. This payment is for the travel services described in the invoice, including but not limited to flights, accommodations, tours, travel insurance, and other related services I have authorized.

By submitting this authorization, I agree to the following:

✔ Payment Authorization
I am an authorized user of the credit card provided and approve this charge. I will not dispute the payment with my credit card company as long as the transaction matches the terms outlined in this invoice.

✔ Refunds & Cancellations
I understand that all payments are subject to Lez Go Travel’s Cancellation Policy, available within our Terms & Conditions. I agree to review this policy prior to booking and understand that refunds, returns, and cancellations are handled accordingly.

✔ Late Payment Policy
I understand that late payments are subject to a $75.00 late fee per occurrence, added to my reservation balance. This fee must be paid in addition to my monthly payment. If no payment is received within 45 days, my reservation may be canceled and all previous payments forfeited.

✔ Name Change Policy
Name changes are permitted up until the final payment date for a $100 fee, plus any additional vendor-imposed charges. After the final payment date, name changes are not guaranteed.

✔ Agency Role
I acknowledge that Lez Go Travel acts only as an agent for independent travel suppliers and is not responsible for their actions, performance, or service. Lez Go Travel has taken reasonable steps to work with reputable vendors but cannot guarantee their services.

✔ Chargeback Policy
If I have a dispute, I will first contact Lez Go Travel within 30 days of receiving the invoice to resolve the issue. I understand that initiating a chargeback without contacting Lez Go Travel may result in additional fees, including legal or collection costs.
