For Non-Website Supplier Purchases

Thank you for your purchase. Virtue Travel, LLC d/b/a FanTabulous Getaways! ("Agency") is pleased to assist in arranging the travel services described in your itinerary, invoice, quote, reservation confirmation, or related travel documents. To complete your transaction and confirm your arrangements, your signature on this authorization is required.

This authorization is not required for electronic purchases you independently complete through our website, www.fantabulousgetaways.com, or affiliated booking platforms.

The travel services being purchased are provided by independent third-party suppliers, including but not limited to hotels, resorts, cruise lines, tour operators, airlines, transportation providers, vacation package providers, and other travel vendors ("Suppliers"). All charges authorized under this agreement are payable directly to the applicable Supplier. Independent Travel Advisors affiliated with Virtue Travel, LLC d/b/a FanTabulous Getaways! may not accept or process supplier charges through any other account and may not accept cash, checks, or other forms of payment in lieu of a supplier credit card authorization unless otherwise expressly authorized by Agency policy.

By signing below, I authorize Virtue Travel, LLC d/b/a FanTabulous Getaways! and/or the applicable Supplier(s) to charge my credit card, debit card, or other approved payment method for the amount specified in my reservation, invoice, itinerary, quote, or subsequent written authorization. I certify that I am the authorized cardholder or account holder and that I have the legal authority to approve the transaction.

I acknowledge that travel prices, availability, taxes, government fees, fuel surcharges, resort fees, gratuities, currency exchange rates, and Supplier-imposed charges are subject to change until fully paid and confirmed. I understand that quoted pricing and availability cannot be guaranteed until deposits and/or final payments have been processed and accepted by the applicable Supplier.

Supplier Terms and Conditions

I understand that all reservations are subject to the terms, conditions, rules, restrictions, and cancellation policies of the applicable Supplier(s). Refund eligibility, credits, exchanges, future travel certificates, and cancellation penalties are determined solely by the Supplier and may vary by reservation, fare type, sailing, accommodation, package, promotion, or service purchased.

Virtue Travel, LLC d/b/a FanTabulous Getaways! acts solely as an intermediary between travelers and Suppliers and does not own, operate, manage, control, or provide the travel services purchased. The Agency is not responsible for the acts, omissions, errors, delays, cancellations, bankruptcies, insolvencies, service interruptions, schedule changes, overbooking, or other failures of any Supplier.

If a Supplier is unable to provide the purchased travel services, any available refund, credit, reimbursement, or compensation shall be governed by the Supplier's policies and the applicable laws or regulations. The Agency does not guarantee the performance of any Supplier and is not responsible for reimbursing losses resulting from Supplier actions or failures.

Payment Disputes and Chargebacks

I agree that I will review all reservation details upon receipt and promptly notify the Agency of any discrepancies or concerns. To the extent the travel arrangements have been booked and performed in accordance with the authorized reservation details and Supplier terms, I agree not to initiate a chargeback, payment reversal, or credit card dispute without first contacting the Agency and providing a reasonable opportunity to investigate and resolve the matter.

I understand that chargebacks filed for services properly authorized and delivered may result in the cancellation of reservations, forfeiture of travel arrangements, collection efforts, and additional administrative costs where permitted by law.

Privacy and Data Protection

Virtue Travel, LLC d/b/a FanTabulous Getaways! will use the personal information provided on this form solely for the purpose of arranging, confirming, and processing your requested travel purchase. We retain personally identifiable information only as long as necessary to fulfill the transaction, comply with legal obligations, resolve disputes, and enforce agreements.

We do not sell or share your personal information with unrelated third parties. Information may be provided only to travel Suppliers, payment processors, insurance providers (if applicable), or other service providers directly involved in fulfilling your travel arrangements and only to the extent necessary to complete the transaction.

Following completion of your travel, personal information will be securely destroyed or retained only as required by applicable law and for the minimum period required by such legal obligations.

Travel Insurance Acknowledgement and Waiver

I understand that travel insurance may provide valuable coverage for trip cancellation, trip interruption, travel delays, baggage loss, medical expenses, emergency medical treatment, emergency evacuation, supplier default, and other unforeseen events.

I acknowledge that Virtue Travel, LLC d/b/a FanTabulous Getaways! has advised and recommended that I consider purchasing travel insurance to protect my travel investment and reduce financial risk associated with unforeseen circumstances.

If I decline travel insurance, I understand and agree that:

• I may be personally responsible for all cancellation penalties, change fees, supplier-imposed charges, travel interruptions, medical expenses, evacuation costs, and other losses that may arise before or during travel.
• Refunds may be limited or unavailable under the applicable Supplier's cancellation policies.
• Neither Virtue Travel, LLC d/b/a FanTabulous Getaways! nor its Independent Travel Advisors will be responsible for reimbursing any losses, expenses, penalties, claims, damages, or costs that could have been covered by a travel insurance policy.
• I voluntarily assume all financial risks associated with declining travel insurance coverage.

By signing below, I acknowledge that travel insurance has been offered and explained to me and that I have had the opportunity to purchase such coverage.

Authorization

By signing below, I acknowledge that I have read, understood, and voluntarily agreed to the terms of this Travel Purchase Authorization, including the Supplier Terms and Conditions, Payment Authorization, Payment Dispute Policy, Privacy Notice, and Travel Insurance Acknowledgement and Waiver.
