I hereby authorize "Breathe Again Adventures" to charge the credit card on this form for the travel related expenses, including flights, accommodations, transportation, tours/activities, planning fees, CC processing and supplier rates.  

I am the authorized cardholder for the credit card provided, and I have the legal right to authorize charges on this card.  

Prior to each transaction, "Breathe Again Adventures" will provide a copy of the invoice detailing the specific charges.  I authorize "Breathe Again Adventures"to charge the specified amount to my credit card for travel related expenses.

I understand that this authorization may be used for multiple transactions as needed by "Breathe Again Adventures".  This authorization remain valid until explicitly revoked in writing or within 7 days of the trip return date.  

I understand that my credit card information will be handled with the utmost confidentiality and in compliance with applicable data protection laws.
