CREDIT CARD AUTHORIZATION AGREEMENT 

By submitting this form, I authorize Melanin Ultimate Travels to charge the credit card provided for travel services related to my reservation. Authorized charges may include hotel accommodations, flights, cruises, vacation packages, optional excursions, shared/ private transportation, and any applicable non-refundable Melanin Ultimate Travels planning or booking fees associated with my travel arrangements.

This authorization will be kept on file and used only for the services outlined on my invoice or travel confirmation. Charges will be applied only for the travel services included in my confirmed booking.

I understand that travel insurance is not included in my package unless selected. I may choose to add travel protection through my vacation package or purchase it independently. If I decline travel protection through my booking, I understand that I have 21 days from the date the trip is secured to purchase travel insurance on my own and submit to Melanin Ultimate Travels.

I acknowledge that Melanin Ultimate Travels and its travel partners do not issue refunds. In the event of a cancellation, any potential reimbursement must be pursued through the third-party travel insurance provider, according to the provider’s terms and conditions.

I understand that charges related to my travel reservation may appear from Melanin Ultimate Travels and/or affiliated travel partners (vendors). Records confirming my authorization and payment submissions will be maintained and provided upon request.

Completing and submitting this form does not automatically process a charge; however, it authorizes the travel advisor to charge the credit card listed for the amounts and purposes approved in my booking or invoice.

I agree to submit a new Credit Card Authorization Form if my credit card information changes before my final payment is completed. If any scheduled payment cannot be processed, I agree to contact Melanin Ultimate Travels immediately to provide an alternative form of payment in order to avoid delays, cancellation, or penalties associated with my reservation.

Chargeback Protection Clause 

I acknowledge and agree that all payments made to Melanin Ultimate Travels are for professional travel planning services and/or travel arrangements secured on my behalf with third-party travel suppliers. By authorizing this payment, I confirm that I am the authorized cardholder and that I approve all charges associated with my booking.

I agree not to initiate a chargeback or payment dispute with my credit card company for services that have been properly authorized and processed in accordance with my booking and invoice. In the event a chargeback or dispute is initiated for a valid and authorized charge, I understand that Melanin Ultimate Travels reserves the right to provide documentation of this authorization, invoice, and booking confirmation to the credit card issuer as evidence of my approval of the transaction.

I also understand that initiating an unjustified chargeback may result in additional administrative fees, collection efforts, and cancellation of my travel reservations, subject to the policies of Melanin Ultimate Travels and all travel suppliers involved.

Travel Supplier Responsibility Disclaimer

I understand that Melanin Ultimate Travels acts solely as a travel advisor and intermediary between clients and independent third-party travel suppliers, including but not limited to airlines, cruise lines, hotels, resorts, tour operators, transportation companies, and excursion providers.

Melanin Ultimate Travels does not own, operate, or control these travel suppliers and therefore cannot be held responsible for any acts, errors, omissions, delays, cancellations, injuries, losses, accidents, or damages that may occur as a result of services provided by these independent suppliers.

All travel services are subject to the terms, conditions, policies, and limitations imposed by the individual travel suppliers.

By booking travel through Melanin Ultimate Travels, I acknowledge that I have read and accept the policies of the travel providers associated with my reservation.
