By submitting this transfer request form, I (the lead guest on the reservation) acknowledge and agree to the following:

1. Eligibility

   • I understand that Sandals and Beaches Resorts allow existing bookings to be transferred to a travel professional only if the reservation is not yet paid in full and was made within the transfer eligibility window (typically within 30 days of original booking).

   • I confirm that my reservation meets these requirements to the best of my knowledge.

2. Authorization

   • I authorize The Agency Chic - Pocono to be added as the travel advisor of record for my Sandals or Beaches booking.

   • I consent to Sandals/Beaches Resorts contacting me to confirm this request, if necessary.

3. Limitations

   • I understand that transferring my booking does not change any existing Sandals/Beaches terms & conditions, including cancellation policies, deposit rules, or change fees.

   • Once transferred, all future booking management, requests, and communications will be handled by The Agency Chic - Pocono in coordination with Sandals/Beaches Resorts.

4. Services Provided

   • After the transfer is confirmed,The Agency Chic - Pocono will provide concierge-style service, which may include room upgrade requests, dining or spa recommendations, excursion assistance, and general travel support.

   • Bonus perks or amenities offered by the agency are separate from Sandals/Beaches Resorts and subject to agency discretion.

5. Acknowledgment

   • By checking the box and submitting this form, I confirm that I am the lead guest on the booking and have the authority to request this transfer.

   • I release Sandals/Beaches Resorts from liability regarding this transfer process and understand all changes remain subject to their policies.
