By signing below, I acknowledge that in calculating the cost of the tour or trip, JodiB Travels LLC and Travelmation, LLC has relied on my consent to these terms below and on their enforceability. In the absence of my agreement to these terms,JodiB Travels LLC and Travelmation, LLC would be unable to offer these services.
PLEASE READ THIS NOTICE. IT CONSTITUTES PART OF YOUR CONTRACT FOR TRAVEL RELATED SERVICES. PLEASE CHECK YOUR DOCUMENTS WHEN YOU RECEIVE THEM. CALL THE TRAVEL AGENCY IF YOU HAVE ANY QUESTIONS. MOST DISCOUNT FARES INVOLVE RESTRICTIONS. CHANGING CARRIERS OR FLIGHTS COULD RESULT IN THE AIRLINE DEMANDING AN INCREASED FARE. CHECK WITH THE AIRLINE OR THE TRAVEL AGENCY BEFORE MAKING ANY CHANGES.
JodiB Travels LLC and Travelmation, LLC is acting as a mere agent for SUPPLIERS (identified on the accompanying documents) in selling travel-related accepting services, or in accepting reservations or bookings for services that are not directly supplied by JodiB Travels LLC and Travelmation, LLC (such as air and ground transportation, hotel accommodations, meals, tours, cruises, etc.). JodiB Travels LLC and Travelmation, LLC, therefore, shall not be responsible for breach of contract, failure to comply with any laws such as the Americans with Disabilities Act (ADA), or any intentional or negligent actions or omissions on the part of such suppliers, which result in any loss, damage, delay, inconvenience or injury to travelers or travelers' companions or group members. Risks, Cancellations and Insurance Traveler assumes complete and full responsibility for, and hereby releases JodiB Travels LLC and Travelmation, LLC from any duty of checking and verifying any and all passport, visa, vaccination, or other entry requirements of each destination, and all safety and security conditions of such destinations, during the length of the proposed travel. It is the passenger’s responsibility to have proper passports or government issued IDs where permitted. For passport and Visa requirements check http://travel.state.gov/travel JodiB Travels LLC and Travelmation, LLC shall not be responsible for any injuries, losses or damages in connection with terrorist activities, social or labor unrest, mechanical or structural integrity of air, sea, and ground transportation and accommodations, diseases, local laws, terrorists acts, bankruptcy or cessation of supplier or carrier services, climatic conditions, Acts of God, delays, changes or cancellation of travel due to weather conditions, hotel services, accidents or health related problems before or while in-transit to (e.g., an accident on the way to a tour), during, and after a tour, or any other actions, omissions, or conditions outside JodiB Travels LLC and Travelmation, LLC control. It is the traveler’s responsibility to protect their purchases and Travel Insurance is strongly recommended. Traveler is advised to obtain appropriate insurance coverage against these risks; information is available through JodiB Travels LLC and Travelmation, LLC regarding travel insurance. Traveler's retention of tickets, reservations, or bookings after issuance shall constitute a consent to the above and an agreement on his/her part to convey the contents hereto to his/her travel companions or group members. By embarking upon his/her travel, the traveler voluntarily assumes all risks involved in such travel, whether expected or unexpected. Traveler is hereby warned of the above risks as well as possible travel industry bankruptcies and medical and climatic disruptions, and the possibility traveler may be unable to travel as scheduled because of personal emergency.
COVID-19 TRAVEL WAIVER
I have requested that JodiB Travels LLC and Travelmation, LLC make one or more travel bookings on my behalf. As the worldwide COVID-19 coronavirus pandemic remains ongoing at this time, I acknowledge that for this reason, and other reasons not reasonably foreseeable at this time, these travel plans may be interrupted or cancelled by the supplier that is providing them, a government entity or other third party over which Agency has no control. I further acknowledge that the supplier’s own cancellation, rebooking and refund policies, subject to any applicable law that is now or may later be in effect, will govern my rights and remedies, including my right to receive a refund, in such an event. Moreover, I understand that should I elect to purchase travel insurance, the terms of the policy will dictate whether, and to what extent, coverage for any financial loss may exist under the circumstances. By signing below, I hereby agree to hold Agency harmless and release it from any and all liability for any damages, including but not limited to monetary losses, I may incur as a result of such interruption or cancellation of these travel plans.
Vaccination Requirements and Other Rules: We have no responsibility for COVID-19-related requirements that travel suppliers and governments may impose from time to time, such as required vaccinations, health affidavit forms, COVID-19 screenings prior to departure or upon arrival, face coverings, or quarantines. For the latest travel supplier requirements, check the website of each supplier, and keep checking because these rules are constantly changing, and we are not responsible for those changes.

Card Holder Authorization: By submitting this form I, the above-named authorized credit card holder, give JodiB Travels LLC and Travelmation, LLC and its representatives authorization to charge my credit card. I understand that this form constitutes a legally binding contract and that by affixing my signature to this form, I will be held responsible for all agreed upon charges as well as any and all collection and legal fees. This credit card is authorized for charges noted above and additional charges will be approved by me before processing. By signing this "Credit Card Authorization" form, I/my organization acknowledge receipt and understanding of its contents, including all terms.
NOTE: Your card will not be automatically charged by completing this form & will be processed by the agency; however completing and submitting this form does give our company the right to charge the card on file for the amount stated above, along with any other amounts you give in writing (hand, email, text, etc.) to the agent to finalize any/all details of your trip. This card will also be used when accessing or creating your My Disney Experience or any other Supplier account to secure dining, purchase Photo Packages, Spa Treatments, Tee Time, or any other components as requested & approved by the client, unless you indicate "No" in the above questionnaire. All Arrangements secured with this card are subject to a cancellation fee if reservation is a NO-Show & not cancelled within the Supplier's Cancellation Terms & Conditions for the particular Reservation (Resort, Dining, Activity, Etc.)
