By submitting this form, you (the client) are requesting a quote for travel and understand that the booking is not confirmed and finalized until a deposit is paid successfully.

 

All payments are processed manually by Breatta Travels, LLC/Travelmation, Breatta Schmadeke with the exception of any automatic payment as described in the booking terms.

 

Payments are processed within 24 business hours that the booking deposit is requested, or payment request is made by the client and will be charged to the card on file unless otherwise stated by the client. 

 

By submitting this form, you allow Breatta Travels, LLC/Travelmation, Breatta Schmadeke to contact you via email correspondence and telephone communication if necessary. 

 

For record-keeping purposes, email correspondence will be the main form of communication between you (the client) and Breatta Travels, LLC/Travelmation, Breatta Schmadeke. 

 

Hours:

Yes- I love what I do, and this is my full-time career, but like everyone else I enjoy spending time with my family and friends.  My office hours are 7am-4pm CST Monday-Friday, out of office on Saturday and Sunday and major holidays. Any emails received outside of these working hours are subject to my availability and will be returned within 48-business hours. There is always an exception to this is for travel emergencies that may arise for my current traveling clients- in the event of a travel emergency, you will be provided with my emergency line via email prior to departure. Liability, Cancellations, and Changes:

I acknowledge the risk and hazards of travel where injury, delay, or unanticipated events may occur.  By signing below, I accept responsibility for my own welfare and waive any future claims against Breatta Travels, LLC/Travelmation, Breatta Schmadeke for liability.  I understand that upon agreeing on an accepted travel package, I agree to sign a Terms and Conditions including a Liability Waiver. 

Cancellations and changes are subject to the policies and fees of the resort, hotel, airline, and travel insurance. Breatta Travels, LLC/Travelmation, Breatta Schmadeke assumes no liability for costs incurred to cancellations and changes/modifications including those related to COVID-19 or changes due to client providing incorrect information at the time of booking. These items are including but not limited to invalid name or date of birth provided to the agent and changes outside of the agent's control (i.e.. itinerary changes by the vendor/resort, change in included amenities, etc.) 

It is very important that you provide accurate demographic (name and date of birth as it appears on identification/passport) as some entities will require a fee and or cancellation of originally booked services in order to change this information once a deposit is made. 

Passports must be valid for 6 months after your planned arrival back to the United States and cannot be issued for some destinations within 1-3 months of arrival. You may be denied travel if your passport expires within this time frame or if you have recently been issued a new passport. 

Travel Insurance:

The purchase of travel insurance is strongly encouraged for all clients and a form denying coverage must be signed if you decline.
