Submission of this form allows Carl Mataushek and Travelmation, LLC to contact you by email or phone.
Terms of Booking
Your submission for a quote request agrees that you understand that a booking is not final and cost of the trip is not locked in or set in stone until a deposit is paid. Deposits MUST be submitted by 6pm EST. A trip is not secured without a paid deposit and confirmation email. Pricing is subject to change at any time prior to deposit payment.
Prior to Departure
You agree to supply Carl Mataushek, with all things requested for booking. These things may include an information form, a copy of your passport, or a photo of your ID. You agree to pay your deposit and final balance on time. You agree that all information provided to Carl Mataushek, Travelmation, LLC is accurate. If you provide any false or incorrect information such as name or birthday to Carl Mataushek, Travelmation, LLC you are responsible for any consequences or fees that may incur.
My office hours are 9am-6pm, M-F. Any emails or messages received on evenings and weekends are subject to my availability.
Room/mate Changes
If you make a change to your room type or amount of people in your room, you may have a new total or a change fee depending on the vendor. You understand that your room price is based on the date of change, not the original booking, meaning it will most likely will be higher. You understand that removing a person at any time from a room will cause a higher cost for the remaining party. With cruises, rooms categories for occupancy varies and availability depending on occupancy varies.
Changes and cancellations are subject to resort/hotel/cruise line/airline fees, and travel insurance. Carl Mataushek + Travelmation assume no liability for costs incurred due to changes and cancellations.
Passports must be valid for 6 months after your planned arrival back to the United States. You may be denied travel if your passport expires within this time frame. Upon booking, you agree to these terms, and will be responsible for all costs associated with passport issues if they do arise.
The purchase of travel insurance is encouraged and a waiver must be signed if declined.
Carl Mataushek and Travelmation, LLC is not legally liable for costs incurred for changes or COVID-19.
