Disney Vacation Request Booking Form
Ready to Book? Please fill out the form below with your details. You will need to complete a separate travel form for EACH destination. Once the completed travel form has been received, Shades of Blue Travel will send you an invoice for the research fee (according to the research fee structure listed below) along with a credit card authorization form. Upon receipt of the research fee payment, Shades of Blue Travel will begin working on your trip. Please allow up to 72 hours for your initial quote after payment has been received.
Shades of Blue Travel does charge a non-refundable research & planning fee. Please refer to the research fee structure below for pricing.
SERVICE AGREEMENT: To begin working together on your trip we require a research fee. This fee is implemented to cover the time spent researching, planning, customizing, and negotiating the best vacation to fit your request/accommodations. You will receive 1 (one) destination of your choice, 2 (two) range of dates within the same travel month, and up to 2 (two) additional quotes. Please see research fee structure below for pricing. **This fee is separate from the cost of your trip and is non-refundable.

Research Fee Structure
Initial Consultation Fee (under 4 guests): $75
Group Travel Fee (over 5 guests): $125+
Group Travel Fee (over 15 guests): $300+
Additional Services and Fees
Destination Wedding/Honeymoon Planning Services: $375
Change Fee: $25+ in addition to supplier penalties.
Trip Cancellation Fee: $45, in addition to supplier penalties.

CLIENT RESPONSIBILITIES: Travel documents including passports visas and health/vaccination certificates required are your responsibility! The name, date of birth and gender that appears on the identification card must exactly match the same such data that is listed on airline ticket(s) and booking records. Additionally, a visa and vaccinations may be required.
